Overview

The first webinar in a series of 3 will explore how human behaviour, workplace culture and organisational systems contribute to accident prevention. The session will begin with an overview of the latest HSENI accident and incident statistics, highlighting current trends and areas of concern across the industry.

We will then explore the key human factors that often sit behind workplace incidents; including decision‑making, communication, fatigue, risk perception and organisational pressures. The discussion will focus on how understanding these factors can strengthen incident investigations and lead to more effective controls.

The webinar will also look at how the industry has moved beyond simply recording accidents and near misses, towards using leading indicators to predict and prevent incidents before they occur. Delegates will gain insight into practical tools, examples and approaches that can support this proactive shift.

The format will include a structured presentation followed by a Q&A session, giving attendees the chance to ask questions and share experiences from their own workplaces.

Liam Baxter is a Chartered Civil Engineer, has a diploma in Health & Safety and is a Chartered Member of IOSH. Liam is an Associate in the Construction Design Management (CDM) Team with over 20 years’ experience initially on a wide range of civil engineering projects and for the last 8 years has specialised in CDM.  His CDM experience includes Principal Designer and PSDP (on ROI projects) for a wide variety of works such as hospitals, marine works, infrastructure, commercial office developments, education and airports.  Recent key Principal Designer roles which Liam has been involved include Carlingford Lough Greenway, the Greenlink Interconnector and Altnagelvin Hospital Restaurant.

Ash is an inspector with the Rail Accident Investigation Branch (RAIB) with 5 years’ experience in this role. Recently, Ash has led investigations into a passenger fatality on London Underground, a passenger train derailment at Grange-over-Sands, and a freight train runaway at Kineton.  Ash is also Fellow of the Chartered Institute of Railway Operators (CIRO).

Bernard Grainger

Bernard Grainger is SQE manager with Babcock Rail.

He has been in Construction Safety for 23 years most of that with Babcock in track renewals and civil engineering in GB and he was the Babcock representative on the Track Safety Alliance Work at Height Group or a number of years.

Bernard has been the Babcock Rail Safety lead in Northern Ireland on Translink Frameworks and Projects since 2019.
